The aim is to revolutionise the way 220,000 residents of Trafford are cared for by embarking on an ambitious Project to join up patient care via the first-of-its-kind Patient Care Co-ordination Centre (PCCC). David will explain how CSC are working in partnership with the NHS Trafford Clinical Commissioning Group to provide a new end-to-end care model that works like an air- traffic control system, tracking patients as they move through the system, and guiding them to different services in an efficient manner. This includes initial patient communications (through a unified communications system), through to clinical workflow based on a catalogue of services, to back-end analysis of the effectiveness and efficiency of care delivery. Results include improved patient experience and enhanced system efficiency, better co-ordination of services across Primary, Secondary and Tertiary care settings, cost savings, reduced bureaucracy and improved value for money. David will be happy to answer your questions on how the PCCC works.
David Lee has over 27 years’ experience as an NHS GP and also holds an MBA qualification. He has combined his clinical work as a GP with medical leadership roles and consultancy. His skills include providing expert advice on Primary and Urgent Care, clinical facilitation and engagement, quality improvement within healthcare systems, coaching, leadership, and supporting personal, professional and organisational change.
